Fee & Inclusive Services

The fee for an appointment with Dr. Anderson is $250 for new patients and $150 for existing patients (plus Hawaii General Excise Tax).

Most insurance companies cover conventional lab testing. Laboratories can bill insurance directly, but this should be done at your discretion. We encourage you to call the customer care number on your insurance card to make sure you are using an in-network lab and that your insurance covers labs ordered by an out-of-network naturopathic physician. 

Functional medicine labs test for nutrient deficiencies, adrenal glands (cortisol), mold, mycotoxins, complete hormone testing, SIBO, heavy metals, environmental toxins, gut microbiome, parasites, and organic acids, and more. Functional medicine labs are cash-pay, but offer the ability to bill through insurance when possible. Laboratory fees are transparent will be discussed during your visit.

Full payment of any outstanding balance is required on the day of the appointment. Acceptable forms of payment are credit/debit cards.

Please provide at least 48 business hours (2 business days) notice for all cancelled or rescheduled appointments. This helps ensure that we can run an efficient and effective practice and meet the needs of other patients who are waiting to get an appointment. There is a $50 fee associated with all no shows, cancellations or rescheduled appointments made within 48 business hours of your original appointment time.

Insurance Coverage

The Mana Clinic does not accept insurance and is out-of-network with all insurance plans. However, some HMSA PPO-type plans reimburse for medically necessary services provided by out-of-network naturopathic doctors. Many of our patients with HMSA PPO-type plans receive a partial reimbursement for their appointment if they have already met their annual deductible. To determine if you are eligible for reimbursement please call the number on the back of your insurance card. 

If you have one of the participating HMSA PPO plans we will prepare a reimbursement claim form for you to submit upon request. In order to receive a reimbursement claim form, you must fill out the insurance information section of the intake form.

Other insurance plan types do not reimburse for services provided by out-of-network doctors such as HMO plans (eg Kaiser), Medicare (including Advantage plans), Medicaid (Aloha Care and Quest plans), and Tricare plans.

For itemized receipts for FSA/HSA/HRA, please request a detailed receipt following your appointment.

No Surprises Act Good Faith Estimate

Per the “No Surprises Act”, here is the Good Faith Estimate of what you could pay for medical services at The Mana Clinic LLC:
For new patients: Service Code 99204 New Patient Visit: Level 4 $250
For existing patients: Service Code 99214 Established Patient Visit: Level 4 $150
This estimate does not apply for services that are not traditionally covered by insurance such as cosmetic services.
